3、投标人资格要求
（1）投标人必须是来自中华人民共和国或是与中华人民共和国有正常贸易往来的国家或地区（以下简称“合格来源国/地区”）的法人或其他组织，在法律上和财务上独立、合法运作并独立于招标人和招标机构，具有相关经营范围，有资格和能力提供本采购项目的货物及服务的制造商或代理商（或经销商）,总公司或者分公司只允许一家投标，不允许同时参与本项目投标，以分公司名义参与投标的，须提供总公司或具有独立法人的上一级公司出具的愿为其参与本项目投标的行为以及履约等行为承担民事责任的加盖总公司公章的授权函，以及提供总、分公司的营业执照扫描件。须提供营业执照或企业注册证明复印件。
（2）参与国际招标的投标人必须在中国国际招标网www.chinabidding.com 注册并进入该网的供应商库中（注册网址：“中国国际招标网（www.chinabidding.com）”，注册方法：请登录网站查询或拨打网站服务咨询电话：010-58851111-8762（客户服务部）、13261902952；传真：010-59713266；联系人：刘杰万），境内投标人同时需具有深圳市政府采购注册供应商资格（供应商注册网址：http：//www.szzfcg.cn，投标文件中无需提供证明材料。采购代理机构将在开标当天从供应商资料库里查询本项目所有投标供应商的注册状态，投标供应商注册状态不是“有效”的，将按资格不符合招标文件要求，作资格审查不通过处理）。
（3）投标人在中华人民共和国境内注册时，若投标人为所投产品的生产企业，必须提供《医疗器械生产企业许可证》，且生产范围包含该产品；若投标人为所投产品的代理商或授权供应商，必须提供《医疗器械经营企业许可（备案）证》，且经营范围包含该产品；
（4）投标人必须提供所投产品的《医疗器械注册（备案）证》的复印件，原件备查（开标时，该证应在有效期内；若不在有效期内，则需提供该证和所投产品在该证有效期内生产的药监局出具的证明文件）；
（5）若投标人所投产品为进口设备，则必须提供所投产品制造商出具的销售授权书，且同时提供由产品制造商或授权的中国总代理签署的合法有效的保修、维修承诺；
（6）投标人须自行承诺，参与本项目投标前三年内（即从本项目招标公告发布之日起往前推算36个月），在经营活动中没有重大违法记录，以及参与本项目政府采购活动时不存在被有关部门禁止参与政府采购活动且在有效期内的情况；与其他投标供应商不存在单位负责人为同一人或者存在直接控股、管理关系；未对本次采购项目提供整体设计、规范编制或者项目管理、监理、检测等服务。（由供应商在《承诺函》（格式自定）中作出声明）；
（7）参与政府采购项目投标的供应商未被列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单（由供应商在《政府采购投标及履约承诺函》中作出声明）。
